How life insurance approval and pricing work
What insurers look at, how health classes set your price, and how to get the best rate you qualify for.
Short answer: underwriting is how an insurer decides whether to approve you and at what price. It reviews your age, health, lifestyle, and history, then assigns a health class. Better classes mean lower premiums, and different carriers can assign different classes to the same person.
What insurers check
- Application answers about health, medications, and family history
- Prescription history database
- Medical Information Bureau (MIB) records from past applications
- Motor vehicle record
- Medical exam and lab results, when required
- Doctor's records, when needed
- Credit-based insurance score in some states
Health classes
| Class | Typical profile |
|---|---|
| Preferred Plus | Excellent health, ideal weight and blood pressure, no tobacco |
| Preferred | Very good health with minor issues |
| Standard Plus | Good health, a few common conditions under control |
| Standard | Average health |
| Table ratings | Higher-risk conditions, priced above Standard |
| Tobacco | Separate, higher prices for nicotine users |
Exam or no exam
| Path | How it works | Timing |
|---|---|---|
| Accelerated (no exam) | Health questions plus database checks | Same day to a few days |
| Fully underwritten | Paramedical exam, blood and urine tests | 2–6 weeks |
| Simplified issue | Fewer questions, no exam, usually costs more | Days |
| Guaranteed issue | No health questions, small amounts, higher cost | Immediate |
How to get a better rate
- Apply while you are healthy and younger
- Compare carriers: each treats conditions such as diabetes, sleep apnea, or high blood pressure differently
- Schedule the exam in the morning, fasting, and avoid heavy exercise the day before
- Bring a list of medications and doctors
- Answer honestly. Misstatements can lead to a denied claim
- Quit nicotine: most carriers reclassify after 12 months tobacco-free
Agent tip: If you have a health condition, tell us before applying. We can shop it anonymously with several carriers first.
If you are declined or rated
A decline from one carrier does not mean every carrier will decline you. Ask for the reason, then consider another carrier, a smaller amount, simplified issue, or reapplying after the condition improves.
Underwriting: common questions
How long does life insurance approval take?
No-exam approvals can take minutes to a few days. Fully underwritten policies usually take 2 to 6 weeks.
Can I get life insurance with diabetes or high blood pressure?
Often yes, especially if the condition is well controlled. Prices and classes vary widely by carrier.
Does applying hurt my credit?
No. Life insurance applications do not create a hard inquiry on your credit report.
